You are also able to specify if it is either standard/predefined or custom form templates that can be customized as per user needs

Form Name: This is the type of financial statement

Form Title: This is the name of your financial report

General Ledger Year: This is the year the GL was set-up 

Please select your account(s): Allows you to select the accounts including account details  associated with the form for presentation purposes.
